Combined transcatheter therapy of aortic stenosis and thoracic aortic aneurysm.

چکیده
Dear Editor, Transcatheter aortic valve implantation (TAVI) and thoracic endovascular aortic repair (TEVAR) are independently well established techniques for treating aortic valve stenosis and thoracic aortic aneurysms. The co-existence of both lesions in patients occurs as they share similar risk factors but the treatment of patients with these combined lesions and high surgical risk remains unclear. The presence of a thoracic aneurysm was an exclusion criterion in the landmark PARTNER trial.1,2 The presence of such a lesion increases the risks of aortic rupture and signifi cant device interaction may occur during the procedure. There have thus been no prior reports of the feasibility of treating such patients with combined TAVI and TEVAR.

Combined Transapical Transcatheter Aortic Valve Replacement and Thoracic Endovascular Aortic Repair for Severe Aortic Stenosis and Arch Aneurysm.
An 83-year-old male with multiple comorbidities presented with critical aortic stenosis and a saccular aortic arch aneurysm. Through a mini thoracotomy, a balloon expandable transcatheter aortic valve was delivered transapically. A thoracic stent graft was then delivered through the prosthetic valve and deployed in the arch, while a covered stent was deployed in the left common carotid artery. ...
متن کاملTranscatheter Aortic Valve Replacement: A Review Article
Transcatheter aortic valve replacement (TAVR) is a novel therapeutic intervention for the replacement of severely stenotic aortic valves in high-risk patients for standard surgical procedures. Since the initial PARTNER trial results, use of TAVR has been on the rise each year. New delivery methods and different valves have been developed and modified in order to promote the minimally invasive p...
متن کاملMycotic aortic aneurysm as a postsurgical complication: report of a case and review of the literature
Mycotic aneurysms are localized and irreversible dilatations of the arteries caused by weakening and damaging the arterial wall by an invasive organism establishing infective arteritis. Mycotic aneurysm of the thoracic aorta is a rare event; however, it can be fatal if not diagnosed early or not treated appropriately. Clinical findings are usually nonspecific; however, c...
متن کاملBicuspid Aortic Valve: An Unusual Cause of Aneurysm of Left Coronary Sinus of Valsalva
Bicuspid aortic valve is traditionally considered an innocuous congenital anomaly. Due to a better and widespread availability of non-invasive imaging techniques, it has come to the fore that 30% of these cases develop complications, viz., valve abnormality (aortic regurgitation and stenosis), and aneurysm of aortic root and ascending aorta. Sinus of Valsalva aneurysm is an uncommon complicatio...
متن کاملTransapical aortic valve replacement through a chronic apical aneurysm.
Transapical aortic valve replacement through an apical aneurysm is traditionally contraindicated because of the risk of severe systemic embolization when thrombi are present. However, a chronic fibrotic aneurysm without apical thrombi carries a low risk of distal embolization and can be safely employed for a transapical transcatheter aortic valve replacement in case of absence of an alternative...
